How to access / refer to the service?

The majority of people are referred to our service by a member of staff in the eye clinic. This can be done by bringing the patient along to the worker, or completing a yellow referral form. Stocks of these are in each eye clinic. We are happy to receive self-referrals and referrals from family friends and carers. You can approach our workers directly, check the timetable below for who is at what hospital and when.
